fix: catch concatenated name forms (JohnSmith, john.smith, etc.)

Smart patterns now catch all concatenated combinations of first+last
names with common separators:

Case is preserved: all-lowercase input → lowercase output,
ALL-UPPERCASE → uppercase, mixed case → as configured.

Also bumped all versions to 2.0.5.
