docs: fix Phase 3 BIOS table for newer R730xd firmware

VT for Direct I/O toggle and Hard Disk Drive Sequencing were removed
from BIOS 2.19+; VT-d is enabled by default. Add dmesg verification
step and clarify C-states should be disabled. Remove outdated note
pointing users to a nonexistent menu option.
